Care Assistant – Certified, Hospice and Home Care

• Provides personal care for elderly, convalescent or disabled persons in home or facility settings.
• Provides assistance to patients with their activities of daily living.
• Provides personal care for patients as directed and delegated by the nurse, according to the established plan of care.
• Provides services such as housekeeping, laundry, meal preparation, and shopping.
• Assists patient with social activities as prescribed by the plan of care.
• Maintains timely and accurate documentation.
• Reports changes in patient status to supervisor.
• Uses equipment and supplies effectively and efficiently.
• Adheres to environmental, safety and infection policies and procedures.

• Drivers License - Licensing Board Upon Hire
• Certified Nurse Aide (CNA) - Licensing Board If working in the state of Minnesota Upon Hire
• High School or GED Equivalent Certified Nurse Aide (CNA) - Licensing Board
• Less than 1 year in home health care or related health care field

Demonstrates expertise in providing personal care and assistance with daily living activities for elderly and disabled individuals, while adhering to safety and infection control policies. Maintains accurate documentation and effectively communicates changes in patient status to supervisors.
