• Provide direct patient care to residents in a skilled nursing facility*
• Administer medications and treatments as prescribed*
• Monitor patient conditions and report changes to the care team*
• Assist with daily living activities, including hygiene and mobility*
• Maintain accurate and timely documentation*
• Collaborate with RNs, CNAs, and healthcare staff to ensure quality care*
• Support patient comfort, safety, and dignity at all times

• * Active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license*
• * Graduate of an accredited practical nursing program*
• * Current BLS (Basic Life Support) certification*
• * Minimum of 1 year of experience in skilled nursing or long-term care preferred*
• * Strong clinical, communication, and organizational skills*
