Social Worker

A well-established hospital located near Marshall, Minnesota, is seeking a Social Worker to join its exceptional team.

Social Worker Opportunity:

• Full-Time, Permanent, Direct-Hire
• Schedule: 8 am - 4:30 pm
• Reports to Hospital DON and Hospice Manager

Social Worker Responsibilities:

• Be a core member of the Hospice team and provide support to patients and families' emotional, social, and economic needs relating to the impact of a terminal illness
• Plan the transition and transportation of patients from hospitals to a home or another facility, arrange services, and collaborate with doctors, nurses, and insurers
• Plan and lead interdisciplinary meetings, facilitate referrals for community resources, and review critical access admissions and monitor patients with regularly scheduled admission visits

Social Worker

Qualifications:

• Must have a Bachelor's degree in Social Work
• Must have a valid LSW license or the eligibility to obtain one
• Previous experience in Hospice is a bonus
