Self-Sufficiency Case Manager
Help individuals and families build confidence, skills, and stability as a Self-Sufficiency Case Manager. Join our team and be a source of advocacy and support!
• Empower & Support:
Provide case management services that help individuals and families set and achieve personal goals.
• Guide Participants:
Conduct assessments, develop individualized plans, and assist in stabilizing housing situations.
• Manage Cases:
Keep detailed records, document all contacts, and maintain participant confidentiality.
• Promote Self-Sufficiency:
Support participants in becoming independent from public assistance.
• Community Engagement:
Build relationships with service providers, attend networking meetings, and advocate for participants.
• Stay Organized:
Ensure accurate documentation, track referrals, and complete necessary reports on time.
• You're a strong communicator, both written and verbal, who easily connects with people.
• You can assess needs and create action plans that drive real results.
• You have experience working with multiple population types and advocating for individuals.
• Problem-solving and conflict resolution? You handle them with confidence.
• You're a team player who also thrives when working independently.
• You approach cultural and economic differences with sensitivity and understanding.
• Minimum of two (2) - four (4) years of education and/or experience in a related field required.
• A four-year post-secondary degree in Human Services in preferred.
• Starting salary of $20.44 - $31.34/hour + Benefits!
• Full-time (40 hours per week), 12-Months a year
