Position

Service Coordinator, Marshall office.

40 HOURS/WEEK, FULL-TIME POSITION

Sign On Bonus of $500 up front/ and $500 at 6 months

WAGE RANGE $20.50 and up depending on education and experience & EXCELLENT BENEFITS

Responsibilities

This is a summary of the work a Service Coordinator completes regularly at PETTIS COUNTY BOARD OF SERVICES FOR THE DEVELOPMENTALLY DISABLED / MPOWER:

  • Pursues services, supports, and funding for those served to ensure both active treatment and maintenance of skills aimed to aid in progress towards individually identified goals.
  • Collaborates to secure a wide range of information that contributes to the individual's current status in order to develop the Individual Support Plan (ISP).
  • Maintains a working knowledge of and locates, refers, arranges, and accesses community resources available for the individual served.
  • Develops, monitors, and maintains records regarding services rendered and assesses these services in relation to supporting the individual's progress toward ISP goals.
  • Serves as a liaison between multiple agencies and team members to ensure that the individual's needs are being addressed.
  • Organizes and facilitates team meetings to discuss progress and possible modifications to services in the ISP. Works with others in a successful manner.
Qualifications

Must meet requirements set forth by CMS (Center for Medicaid Services) and DMH-DD (Department of Mental Health Division of Developmental Disability) by securing:

  • A Registered Nurse license
  • AND/OR
  • A Bachelors degree from an accredited college or university
Additional Considerations
  • Employee will frequently be required to use multiple programs, platforms, and databases.
  • Employee will be provided with technology that supports a fairly paperless work fashion.
  • Employee will be required to drive and therefore must maintain a valid driver's license and automobile insurance.
  • Employee is regularly required to sit, stand, walk, talk, hear, use hands to type or handle objects, and use balance.
  • Employee will need to have close vision ability and ability to adjust focus while using the computer and/or driving.
  • Employee may occasionally lift and/or move up to 30 pounds.
  • When working in the office, employee may experience quiet to moderate noise level in a temperature-controlled climate of a shared office setting.
  • Employee may experience a moderate level of stress while working in this position.
  • Employee will have the opportunity to manage their own schedule, work at the office or from alternate locations, and will receive reimbursement for travel, training, and overnight accommodations while on the clock.
