Chaplain - Part Time

Qualifications

• Bachelor’s degree in counseling, theology, divinity, or a related field preferred, or be an ordained clergy person with Clinical Pastoral Education (CPE).
• Minimum of two (2) years of experience in hospice pastoral care, chaplaincy, or an equivalent setting preferred.
• Demonstrated knowledge of and sensitivity to diverse theological perspectives, spiritual traditions, rituals, customs, and beliefs.
• Strong interpersonal, organizational, and communication skills, with the ability to provide compassionate support to patients, families, and interdisciplinary team members.
• Ability to collaborate effectively within a multidisciplinary hospice team and with community faith leaders and organizations.

• Complete and document spiritual assessments for patients and families within five (5) days of admission, ensuring care aligns with identified needs.
• Provide ongoing spiritual and pastoral support based on the wishes, beliefs, and needs of patients and families.
• Offer spiritual care visits to all patients and serve as a resource to the interdisciplinary team when visits are declined or additional support is needed.
• Develop and contribute to individualized plans of care in collaboration with patients, families, the interdisciplinary team, and community clergy when requested.
• Conduct or facilitate prayer, worship, rituals, and other spiritual services as appropriate.
• Support individuals of diverse spiritual, religious, and philosophical backgrounds in exploring beliefs, values, and concerns.
• Document spiritual care and counseling in the patient’s clinical record in a timely and accurate manner according to organizational policy and regulatory requirements.
• Collaborate with the interdisciplinary team to address spiritual, ethical, and pastoral concerns and evaluate patient and family response to care.
• Provide consultation, education, and support to hospice staff, including bereavement and crisis support when requested.
• Participate in interdisciplinary team meetings, care planning, staff meetings, and required in-service education.

• Collaborate with the interdisciplinary hospice team, including nurses, physicians, social workers, volunteers, and administrators, to support holistic, patient-centered care.
• Communicate effectively with patients, families, staff, and community clergy or spiritual care providers to ensure care aligns with patient wishes, beliefs, and goals.
• Participate in interdisciplinary team meetings to share spiritual assessments, contribute to care planning, and support continuity of care.
• Serve as a spiritual care resource to team members by offering guidance, consultation, and education related to spiritual, cultural, and ethical considerations.
• Engage in ongoing professional development and participate in required trainings to maintain competency and best practices in hospice spiritual care.
