Primary-Health-Care, Inc. seeks a Registered Nurse to join our Ames/Marshalltown care team as Nurse Care Manager. You will implement risk stratification, manage chronic conditions, and coordinate holistic care for patients with complex needs.
The role is based at the Marshalltown Clinic, Monday–Friday 8 AM–5 PM, with occasional travel to other PHC sites. Mileage reimbursement and travel bonus may apply as part of our PHC iCare program.
