Social Worker 3 - Licensed Social Worker Preferred
The Social Worker 3 position at the Iowa Veterans Home will have the honor to serve a caseload of approximately 60 Veterans, spouses and families, directly assisting with providing the highest quality of life in a home‑like long term care setting.
The Social Worker is part of a small unit interdisciplinary team serving residents in two locations, as well as part of a larger team of our social work group ensuring the Iowa Veterans Home consistently provides exceptional quality of care to all that live with us every day! The social workers at the Iowa Veterans Home have the opportunity to serve veterans and families, honoring the service they have all provided.
The Iowa Veterans Home is a 400-bed facility that provides many different opportunities. We are similar to a small community with several activity centers, a bank, a coffee shop and daily off‑grounds recreation trips.
Primary duties will include admission of new residents, support with adjustment to long term care placement, assessments, problem identification and goal setting, and discharge planning. You will work with the resident, family members and other professional and nonprofessional staff and assist with solving personal, emotional, interpersonal and health problems. You will make recommendations and assist residents with planning passes or furloughs.
You will also work with residents and their next‑of‑kin and/or legal guardians regarding residents' rights, advance directives, notification of changes, and other regulatory required communications. Resident and family communication are a regulatory requirement and staff work diligently to open and maintain this connection.
This position will provide direct casework services to assigned residents and families. Preference will be given to those who have obtained LBSW certification. Come join our Social Work team as we make a difference in resident and family’s quality of life here at IVH!
