American Traveler is seeking a Physical Therapist for a Home Health position requiring 1+ year of recent experience and proficiency in HCHB and OASIS.
Requirements- Minimum of 1 year of recent, relevant Home Health PT experience required
- BLS certification required
- OASIS experience required for completing Start of Care (SOC) documentation
- Proficiency in HCHB (Homecare Homebase) required
- Active driver's license and personal transportation required
- License required if applicable
- Strong documentation and communication skills required
- Home Health setting serving a geriatric patient population
- Territory covers up to a 60-mile radius from the hub office, with flexibility required across the territory
- Day shift schedule, 5x8 hours (8:00 AM - 5:00 PM)
- Average caseload of 5-7 patients per day
- On-call rotation required approximately every 3rd to 4th weekend
- Uses HCHB EMR system
- Main responsibilities include home exercise program development and patient/family education
- Treats orthopedic, neurologic, and post-surgical conditions including post-stroke rehabilitation and spinal cord injury
- Scrubs required in any color; jeans not permitted
- iPad provided for documentation; personal cell phone use not required for work purposes
- Mileage reimbursed at the IRS standard rate
- Monday orientation required, including completion of billable modules
- Travel pairs accepted, though each traveler will be assigned to their own territory and must have independent transportation
- Timely documentation and communication are mandatory expectations of the role
- RTO requests must be submitted at the time of application
- Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
- First-time travelers will not be considered for this position
