Job Title and Location

Senior Registered Nurse – Hospice (Full Time) – Green Lake, WI (SSM Health)

SSM Health at Home seeks a dedicated Hospice RN to travel to patient homes and community facilities to deliver end‑of‑life skilled nursing care and compassionate support. This full‑time position requires Monday‑Friday days, 32 hours a week plus on‑call rotations, with a rotating weekend schedule.

• Assess, plan, and perform comprehensive home‑care nursing care, including vitals, wound care, IV access, and medication management.
• Complete admission paperwork, verify insurance, and conduct timely patient assessments.
• Coordinate multidisciplinary team communication, provide physician updates, and document all visits in compliance with regulations.
• Respond to patient and family calls, perform follow‑up, and maintain accurate, timely documentation.
• Maintain patient safety, work in a constant state of alertness, and perform other duties as assigned.

• Graduate of an accredited nursing school or education equivalency for RN licensure.
• Current state RN license in the applicable state (Illinois, Missouri, Oklahoma, Wisconsin) and Basic Life Support (BLS HCP).
• Valid motor vehicle insurance and the ability to travel to patient homes and facilities.
• Strong verbal, written, and interpersonal communication skills.

• No prior hospice experience required.
• Proof of motor vehicle liability insurance upon hire.

• Constant standing, walking, and frequent lifting of up to 25 lb; occasional lifting of 25‑50 lb.
• Work 32 hours per week (Monday‑Friday), with on‑call rotations and a rotating every‑third‑weekend schedule.
