Position Summary
The Inpatient Unit (IPU) Registered Nurse Case Manager is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation, promoting the Compassus philosophy using the 6 Pillars of Success as the foundation. S/he upholds the Code of Ethical Conduct and fosters positive relationships internally and externally. As an IDT member, s/he provides routine and emergency patient assessments, educates patients and caregivers, and coordinates care to meet the needs of patients and families.
On‑call responsibilities and after‑hours emergency support are mandatory. Must apply state and federal regulations.
- Coordinate clinical, psycho‑social, and spiritual services specified in the Plan of Care.
- Collaborate with IDT to address patient needs.
- Explain hospice services, Medicare benefits, informed consent, and elections of benefits.
- Submit documentation and paperwork at completion of visits.
- Identify need for IDT intervention.
- Communicate patient and family needs to IDT.
- Supervise Hospice Aides and LPN/LVN; adjust assignments to promote patient outcomes.
- Communicate with IDT, Medical Director, and Attending Physician regarding symptom management and monitor response.
- Provide physical and emotional care during crisis via case management.
- Coordinate hospice care to ensure appropriateness, continuity, and quality.
- Ensure bedside documentation.
- Maintain updated care plans reflecting problems, goals, and interventions.
- Keep current pharmacological and non‑pharmacological knowledge; apply guidelines and standards.
- Complete routine and emergency assessments; participate in on‑call services.
- Record observations and interventions accurately.
- Prevent unwanted hospitalizations by addressing patient and family needs at home.
- Keep Medical Director and physicians informed of changes and request interventions.
- Perform other duties as assigned.
- Bachelor’s or Associate’s degree in Nursing (required).
- Minimum one year nursing practice or equivalent experience (preferred).
- Experience in hospice, oncology, home health, and/or long‑term care (preferred).
- Active RN license in state(s) of employment (required).
- Current CPR certification (required).
