- Provide home health visits as assigned by the Clinical Services Manager, including patient assessment, admissions, care planning, implementation, and evaluation
- Assess patients’ physical, psychological, social, and spiritual needs and coordinate care with the interdisciplinary team
- Communicate with physicians and other healthcare professionals to ensure optimal patient care
- Perform visits to evaluate patient needs and update plans of care accordingly
- Deliver therapeutic, preventative, and rehabilitative nursing procedures as directed by the physician
- Maintain accurate and timely documentation, including patient records, progress notes, and care plans
- Provide clinical pronouncement of death and support families when appropriate to licensure
- Current Registered Nurse (RN) license in the state of practice
- Graduate of an accredited nursing program
- At least one (1) year of clinical nursing experience (ICU, medical-surgical, or rehabilitation preferred)
- Home health experience preferred
- Current CPR certification
- Reliable transportation, valid driver’s license, and current automobile insurance
