Position: Coordinated Family Support (CFS) Specialist

Coordinated Family Support (CFS) Specialist

The Coordinated Family Supports (CFS) Specialist is responsible for providing comprehensive case management and support services to adults with intellectual and developmental disabilities who choose to remain in the family home. This role involves coordinating access to resources, facilitating service delivery, and supporting long-term planning to promote stability and independence. The CFS Specialist collaborates with families, service providers, and community agencies to ensure individualized, culturally responsive support across Contra Costa and potentially Alameda Counties.

The CFS Specialist is responsible for identifying and coordinating supports for individuals with I/DD to remain in the family home. This includes conducting intake, assessment, and service planning, assisting with navigation of public services, facilitating education related to diagnosis and needs, coordinating consistency and quality of services from support professionals, assisting with appointment scheduling and coordination, identifying transportation and backup services as needed, supporting long-term planning including for those with aging caregivers, delivering training to promote independence and inclusion, ensuring services are culturally and linguistically appropriate, preparing and submitting quarterly service progress reports, reporting special incidents and suspected abuse as required, reporting abuse within 24 hours and notifying supervisors of significant client changes, and performing general administrative tasks as assigned.

The CFS Specialist must maintain Thrive's stability and reputation through professional conduct, follow all applicable policies, procedures, and laws, practice strong communication and support team efforts, and possess understanding of developmental disabilities, mental health, and adult protective services. They should also have strong organizational, record-keeping, and analytical skills, be proficient in conflict resolution and community navigation, and have effective interpersonal and team communication.

Benefits Package Available For All Work Levels. Benefits may vary based on your job level. Full benefits package up to $12,000.

Candidates that tend to do well as CFS Specialists can come from the following backgrounds: case manager, IHSS, IHSS provider, in-home support services, caregiver, home health aide, personal attendant, direct support professional (DSP), paraprofessional, behavioral therapist or anyone that has worked with individuals with developmental disabilities such as intellectual disabilities, autism, cerebral palsy and downs syndrome etc.
