There is currently one (1) vacancy in Health, Housing and Homeless (H3) located in Concord, CA. Incumbents may be required to occasionally travel to different locations throughout Contra Costa County.

Health, Housing & Homeless Services (H3) leads the County’s systemwide response to homelessness, including administration of the Continuum of Care (CoC) program and the County’s Homeless Information Management System (HMIS). Additionally, this program has oversight of emergency shelter programs, transitional housing, rapid rehousing, supportive housing, California Advancing and Innovating Medi-Cal (CalAIM) services.

Under general direction, the Continuum of Care (CoC) Administrator is responsible for the overall administration of Contra Costa’s homeless Continuum of Care (Richmond/Contra Costa CA-505). The Administrator will manage the planning, coordination, and implementation of community-wide efforts towards preventing and ending homelessness as outlined in Contra Costa County’s strategic plan and as identified in the annual Continuum of Care’s workplan.

The CoC Administrator partner closely and work alongside governmental and non-governmental leadership, local, state, and national advocacy groups, homeless service providers, and the Contra Costa Council on Homelessness to support data driven decision making on policies and practices to address homelessness and ensure the effective coordination of homeless services throughout the Continuum of Care.
