Shift: Days, 8a-5p, 8x5, 40hrs per week, Mon-Fri

Length: 13 weeks

Requirements:

• CA License
• 1-year recent (within the last 3 years) full-time home health experience
• BLS

Other Details:

• 4 days visiting patients (5 visits a day) and 1 day per week in office triaging and completing administrative tasks for the nurse practitioners. charting is clinic charting vs oasis as in home health. Basic assessment and reporting an SBAR to the practitioner or MD with results of visits.
• Coverage Area:
  Will work onsite in Martinez and visit patients in their homes, SNFs or Assisted Living.
