Gentiva’s Heartland Hospice is seeking a Patient Care Manager to lead clinical excellence, oversee patient care, coordinate interdisciplinary teams, and ensure compliant hospice services delivered in patients’ homes.
Reporting to the Executive Director or Administrator, you will mentor staff, manage visits, ensure regulatory standards, and drive quality improvement across the location. An active RN license and 3+ years of direct patient care (including hospice/home health) are required.
