- LPN
- Licensed Practical Nurse
- State Licensure Upon Hire Required - DL NUMBER
- Driver License, Valid and in State Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation. Upon Hire Required - CPR
- Cardiac Pulmonary Resuscitation Upon Hire Required
