Roles & Responsibilities   Provide skilled physical therapy evaluations and develop individualized treatment plans
Deliver therapeutic interventions including exercise, manual therapy and functional training
Track and document patient progress clearly and timely in the medical record
Collaborate with nurses, physicians and rehab staff to coordinate excellent patient care
Educate patients and families on home programs and safe functional strategies
Contribute to a positive, patient-centered environment that promotes recovery
Qualifications   Active or eligible for Physical Therapist licensure in West Virginia
Degree in Physical Therapy (DPT or equivalent preferred)
Current CPR/BLS certification (or willingness to obtain)
Experience in rehabilitation or outpatient/skilled settings is a plus
Strong communication skills and ability to work collaboratively with a care team
Education   Doctor of Physical Therapy (DPT) preferred;
Bachelor's or Master's in Physical Therapy considered
Knowledge & Skills   Solid understanding of therapeutic exercise, musculoskeletal and neuromuscular rehab
Skilled in patient assessment, gait training and functional mobility interventions
Comfortable with documentation and basic EMR workflows
Ability to prioritize caseloads and manage time effectively
Compassionate patient education and family communication skills
