The Polytrauma/Pain Management Registered Nurse (RN) Case Manager provides leadership to clinical support team for interprofessional care of patients with complex pain-related needs and intuitively identifies primary source of patients' problems based on knowledge and experience with patients experiencing pain.
The Polytrauma/Pain Management RN Case Manager functions as team leader, progressively assuming charge nurse responsibilities as assigned, under supervision of the Nurse Manager or designee. Functions and responsibilities include but are not limited to:
* Organizing and directing team members to achieve desirable patient outcomes through day-to-day case management.
* Assists with development, assessment, implementation, and monitoring/evaluation of comprehensive plans of care to ensure efficient, effective, and coordinated care in meeting and promoting standards of wellness.
* Coordinates an interdisciplinary approach to assure physical, psycho-social, and environmental needs and resource delivery to achieve patient goals.
* Acts as an advocate seeking and coordinating solutions for self and members of the team.
* Collaborates with the Pain Team to identify ongoing patient needs in the outpatient setting and provides interventions to reduce future hospitalization visit in collaboration with the Primary Care Team.
* Assisting with patient care coordination, education and scheduling for Interventional Pain Patients.
* Complete interventional radiology suite competencies to assist provider with procedures as needed and when available.
* Leads clinical support group in evidence-based quality improvement activities related to pain management and measuring outcomes.
* Other duties as assigned.
