Registered Nurse - Community Care Coordinator

The Community Care Coordinator Registered Nurse is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care.

Duties include but are not limited to;

* Responsible for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

* Executes a streamlined approach to receiving, triaging, and directing timely, coordinated care.

* Uses nursing knowledge and systems theory to assess, organize, facilitate, and guide Veterans through their full range of care options

* Supports the medical center's mission to improve timely access to care, ensure Veterans who want to receive care in the community are referred and scheduled timely

* Influences care outcomes by collaborating with members of the interdisciplinary team both within the VA and in the community

* Provides oversight and care coordination for Veterans with community care consults, RCT consults, Hospital Notification EOCs, Community Programs consults, and Home Health consults.

* Serves as a liaison between referring VA providers and specialty services by triaging consults or referrals to confirm the appropriater care is requested.

Pay:
Competitive salary, regular salary increases, potential for performance awards
