Job Overview

This position requires coordination and clinical skills for the comprehensive management of critical care patients, including biological, social and psychological assessments. The incumbent has specialized skills in patient and family care, initial and ongoing medical management and invasive procedures. They coordinate involvement of medical/surgical specialty teams to develop comprehensive plans, review plans with team members, and participate in guiding that care.

The provider works with the team to ensure adherence to protocol requirements and regulatory guidelines, performs diagnostic and therapeutic procedures, peri‑procedural services, and coordinates discharge planning. The incumbent also participates in the education of residents and medical/nursing students.

Core Duties and Responsibilities
  • Manages comprehensive and individualized care for patients and families in the inpatient/critical care phase of hospitalization.
  • Works with Critical Care staff or follows established protocol to order additional tests or consults for medical management.
  • Performs, interprets and communicates results of diagnostic tests.
  • Consults and communicates with critical care and inter‑professional services regarding abnormal findings during patient evaluations.
  • Develops a comprehensive individualized plan of care that includes teaching, disposition/discharge planning, and necessary referrals.
  • Participates in physician/team rounds.
  • Identifies unstable or complex patient care situations and facilitates appropriate intervention with resources.
  • Recognizes emergency situations and initiates effective emergency care.
  • Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by evaluating individual needs of patients/families.
  • Utilizes basic equipment safely.
  • Facilitates patient discharge disposition or transfer through coordination of appropriate services.
  • Based decisions on scientific principles, established standards of care or research findings.
  • Promotes an environment that facilitates learning, respecting patient/family preferences, priorities, and readiness to learn.
  • Updates own knowledge base throughout the year.
  • Completes all identified clinical competencies.
  • Mentors nursing personnel at all levels of clinical practice.
  • Communicates effectively with patients/families, providers, interdisciplinary teams and ancillary services for coordinated care.
  • Documents the plan of care, interventions and evaluations in a clear and concise manner.
  • Adheres to performance expectations for WVUM employees in People, Service, Performance Improvement, Shared Values & Culture.
  • Performs initial assessment and develops plan for new admissions with attending physician and/or team.
  • Ensures patient summary and orders are current, comprehensive and accurate.
  • Ensures appropriate transition to next team when patient is ready to leave the ICU.
  • Performs appropriate procedures as directed by the Critical Care Team or within scope of practice.
Minimum Qualifications
  • Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing.
  • National Certification by one of the recognized APRN Certification Centers.
  • Licensure or eligibility for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
  • BLS Certification required; additional Advanced Life Support training at the department’s discretion.
  • Fundamentals of Critical Care Support (FCCS) certification required immediately upon hire.
Preferred Qualifications
  • ACGNP certification or ACNP preferred; FNP considered.
  • Acute care and critical care experience preferred.
Physical Requirements
  • Lifting, moving, stooping, reaching, positioning of patients, standing, walking, moving of patients, materials and equipment weighing 40+ pounds.
