Position: Registered Nurse at Magnolia Health Systems Martinsville, IN

ESSENTIAL FUNCTIONS

  • Provide professional nursing care as defined in the Nurse Practice Act and under the direction of the Director of Clinical Services, Patient Care Coordinator, or Medical Director.
  • Communicate appropriate status/condition changes to the DCS, PCC, or MD, per agency guidelines.
  • Perform duties in compliance with Agency policies and procedures and professional and community standards.
  • Participate in coordination of patient care services (hospice and long-term care), attend meetings as required, and follow reporting guidelines.
  • Demonstrate proficiency in use of agency chosen software, EMR, iPad documentation and other systems and technology as required.
  • Ensure documentation is complete, legible and clearly reflects care given, and is completed by the end of the assigned shift or visit.
  • Effectively communicate verbally and in writing/data entry in English.
  • Work in diverse social and economic situations without imposing own values on patient and family, and maintain appropriate boundaries.
  • Receive and process referral information from Intake.
  • Explain hospice care and Keystone Hospice services and/or facility services and programs to prospective patients/families, physicians, and other appropriate individuals.
  • Consult with the Medical Director, attending physician, and/or mid-level provider (if any) on patient’s eligibility for hospice care.
  • Complete all required admission paperwork (e.g. admission packet) and consents for care with patients/families timely and accurately.
  • Conduct and document completion of initial assessments accurately and within required time frames.
  • Provide or collaborate with IDT to provide care needed to address patient/family immediate needs, as identified in the initial assessment.
  • Conduct and document start of/completion of comprehensive assessments accurately and within required time frames for all assigned patients.
  • Coordinate/communicate necessary information with other members of the IDT, consulting physician(s), facility staff, and others as appropriate.
  • Participate in development of the Plan of Care, as appropriate, for each assigned patient.
  • Serve as a resource for all Hospice and/or facility staff to facilitate continuity of patient care and enhance the quality of patient care.
  • Obtain and document physician orders, as necessary.
QUALIFICATIONS AND REQUIREMENTS
  • Registered Nurse with current license to practice in Indiana.
  • Bachelor’s degree in nursing preferred, but not required.
  • Minimum of one (1) year clinical nursing experience; hospice experience preferred, but not required.
  • Demonstrated knowledge of the regulatory requirements for hospice.
TRAVEL REQUIREMENT
  • Travel between long-term care facilities and/or place of primary residence.
  • Attend bi-weekly IDT meetings, if requested.
  • Candidates must be fully Vaccinated against Covid-19, or obtain a religious or medical exemption upon hire.
