RN:
Case Manager
Address:
Martinsville, VA
- Perform comprehensive assessment of high risk members.
- Collaborate with primary care providers to ensure implementation of individualized, comprehensive care plans.
- Address, identify, and continuously reassess cost-efficient, appropriate levels of care.
- Coordinate care transitions with physicians, social workers, and discharge planners.
- Keep detailed records of clinical, functional, and fiscal outcomes.
- Act as patient advocate by protecting privacy and confidentiality issues.
Critical Information M-F 8-5;
On-call schedule: 1 week a month (after hours). Onsite position; contract to hire.
- Associates or Bachelor’s Degree in Nursing or related field.
- Current, unrestricted state Registered Nurse license.
- Case Management certification.
- Full-time employees receive benefits after 90 days of employment.
- 401(k) with company match available after 1 year of service.
