Registered Nurse (RN)
Seeking a dedicated and clinically skilled Registered Nurse (RN) to provide comprehensive home health services to patients throughout Martinsville and Henry County, Virginia. This role requires an independent healthcare professional who can assess, treat, educate, and manage patients in their home environment while coordinating care with physicians and interdisciplinary team members. The ideal candidate will have strong assessment skills, experience with OASIS documentation, and a passion for delivering high-quality patient-centered care.

Monday through Friday, 8:00 AM to 4:30 PM
Weekly
Hours:
40 hours
Pay Rate: $51.00 - $53.00 per hour
Productivity Expectation: 30 points per week
On-Call Requirement:
Rotating weeknight and weekend on-call schedule
Mileage Reimbursement:
Reimbursed between patient visits
- Travel directly to patients' homes to provide skilled nursing services and chronic disease management.
- Perform comprehensive patient assessments and ongoing evaluations to identify healthcare needs and changes in condition.
- Develop, implement, and revise individualized plans of care in collaboration with physicians and the interdisciplinary care team.
- Complete regulatory and clinical documentation, including OASIS assessments, accurately and timely.
- Provide medication management, administration, reconciliation, and patient education.
- Perform wound care treatments and other skilled nursing procedures.
- Educate patients and caregivers on disease processes, treatment plans, medications, safety measures, and self-management strategies.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals to ensure continuity of care.
- Monitor patient outcomes and intervene appropriately to help prevent avoidable hospitalizations and readmissions.
- Maintain compliance with agency policies, state and federal regulations, and home health standards of practice.
- Participate in case conferences, interdisciplinary team meetings, and quality improvement initiatives.
- Active Registered Nurse (RN) license in the Commonwealth of Virginia.
- Graduate of an accredited nursing program.
- Clinical experience in an acute care, skilled nursing, or related healthcare setting.
- Current CPR certification.
- Strong independent assessment and critical thinking skills.
- Valid driver's license with a clean driving record.
- Reliable transportation and proof of automobile insurance.
- Excellent communication, documentation, and organizational skills.
- Ability to work independently in a field-based environment.
- One or more years of home health nursing experience.
- Experience completing OASIS assessments and documentation.
- Familiarity with Homecare Homebase (HCHB) and other EMR platforms.
- Experience with wound care management and IV medication administration.
- Strong care coordination and case management skills.
- Ability to build positive relationships with patients, families, physicians, and interdisciplinary team members.
- Professional clinical attire or scrubs in accordance with agency standards.
- No specific scrub color requirement.
