About The PositionThe Prince George’s County Health Department’s Behavioral Health Services Division is recruiting a Clinical Case Manager (Community Developer I/II). This entry‑level professional role involves community liaison, development, advocacy, and social services work with client populations such as youth, aged, re‑entry, and housing communities. Work includes collecting, analyzing, and summarizing program data under close supervision. Accuracy and compliance with established guidelines are key performance indicators.
Upon successful completion of a probationary period, incumbents may be promoted to a full performance level.
- Links newly released individuals with housing, vocational, and educational opportunities and community services.
- Performs assessments and screenings, then develops an individualized case plan.
- Creates connections and referrals to service providers based on needs and makes warm handoffs.
- Provides ongoing case management.
- Updates the electronic health record database within 24 hours of each interaction or update.
- Models useful skills and effective problem‑solving strategies.
- Shares experiences and provides coaching in regard to employment, training, and/or obtaining housing.
- Supports the client’s engagement in mental health and substance abuse treatment services.
- Provides information on the rights and responsibilities of discharged offenders and satisfies criminal justice system requirements and conditions.
- Provides practical support by accompanying newly released individuals to initial probation meetings, treatment appointments, and employment interviews.
- Makes referrals to community‑based service providers.
- Participates in re‑entry outreach activities.
- Bachelor’s Degree from an accredited college or university in Business, Public Administration, Social Service, Sociology, Psychology, or a closely related field.
- Responsible work experience that includes coordination of group activities, preferably with disadvantaged individuals.
- Must possess and maintain a valid Class C driver’s license.
- LCPC – Licensed Clinical Professional Counselor
- LCADC – Licensed Clinical Alcohol and Drug Counselor
- LGPC – Licensed Graduate Professional Counselor
- LGADC – Licensed Graduate Alcohol and Drug Counselor
- LCSW-C – Licensed Certified Social Worker – Clinical
- LCSW – Licensed Certified Social Worker
- LMSW – Licensed Master’s Social Worker
- LCP – Licensed Clinical Psychologist
- Meet all training and performance standards and demonstrate required proficiency.
- Wear and use agency protective apparel and equipment when applicable.
- Successfully pass pre‑employment checks, including reference checks, background investigations, and drug screenings where applicable.
- Be willing and able to serve as an essential employee; report during standard, non‑standard hours, or emergencies as required.
- Pass a thorough background investigation and possess a valid driver’s license.
