Home Health Physical Therapist - per visit

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a job in Glyndon, Maryland. This is a per diem opportunity (ongoing) with a $10,000 sign-on bonus for full-time consideration. Office services adult and geriatric clients on a per visit basis in territories throughout the Maryland area, including West Baltimore County (Reisterstown, Owings Mills, Randallstown, and Pikesville).

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, including mobility and range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapy, gait training, balance, and other interventions.
• Continuously assess and revise the PT care plan and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate clients, family members, or other representatives in rehabilitative care to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations in client care management, utilizing a state-of-the-art touchpad tablet.

• A current Maryland Physical Therapist license (PT).
• A minimum of one year of recent work experience as a Physical Therapist (PT).
• Graduation from a program approved by the American Physical Therapy Association (APTA) or the Committee on Allied Health Education and Accreditation (CAHEA) of the AMA, as indicated by transcripts or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
