Social Work Assistant

• Full Time Monday-Friday
• 8am-4:30pm
• Long Term Care Experience Preferred
• LSW or CSW Required

• Identifies cognitive impairments, mood problems and other psycho-social needs in residents.
• Maintain accurate, thorough, and timely documentation.
• Review Advanced Directives and provide education to residents and family members as needed.
• Help provide a smooth admission and discharge process for residents and family members.
• Other duties as assigned by Director of SW or facility Administrator

• Prior experience managing staff is preferred.
• Ability to work independently or part of a group.
• Computer knowledge.
• Previous long term care experience is preferred but not required

• Must have active Certificate/Licensure for Social Work CSW LSW
