The Segue Residential Transition Program assists with helping individuals transition out of state psychiatric hospitals. Segue clients are clinically stable and ready to leave the hospital. The services provided assist clients with learning how to manage their illnesses and develop independent living skills.
What to expect.
You will coordinate assessments, community integration activities, planning, linkage, and advocacy
. This position requires general and specific knowledge of crisis stabilization, mental health, community-based programming, and entitlement programs.
Specific responsibilities include:
- Supporting patients and addressing concerns as they arise, including resource coordination, appointments, transportation, skills training, entitlements, and money management.
- Maintaining accurate and complete documentation related to client needs and services provided.
- Implementing appropriate crisis stabilization techniques as necessary.
- Providing direct service for medication monitoring
, including assessment, education, and assistance with filling pill boxes. - Providing transportation to clients using personal or company-owned vehicles.
- Facilitating hospital visits to potential community placements and organizing activities to assess community readiness.
- Participating in program evaluation and continuous quality improvement

This position has a flat pay rate of $20.95/hr.
What we need from you.
- High School Diploma required;
Bachelor’s Degree preferred in related field or work experience that uniquely qualified the individual for the job. - Two years of behavioral health experience within the community.
- Experience with applying for public benefits, including but not limited to Social Security disability benefits, Supplemental Nutritional Assistant Program (SNAP), Temporary Disability Assistance Program (TDAP), and Medical Assistance.
- Working knowledge of Microsoft Office, ability to navigate through applications and utilize keyboard to input required documentation into electronic health record.
- Ability to work independently as well as within various groups.
- Possession of a 4-passenger vehicle that meets SPHS inspection, valid Driver’s License and vehicle insurance that names employee as insured driver. Safe Driving record
