• Provide comprehensive physical therapy assessments and develop individualized treatment plans in a home health setting.
• Deliver skilled therapy services according to physician orders and patient needs.
• Educate patients and families on rehabilitation exercises and health maintenance strategies.
• Document patient progress accurately and in compliance with regulations.
• Collaborate with healthcare teams to ensure coordinated patient care.
