Location: Fallston
Med Star Health seeks a Population Health Dietitian to join the Food Rx program in Baltimore, MD. The role focuses on nutrition counseling, diabetes education, and medically tailored groceries for individuals with diabetes and chronic diseases.
You will collaborate with Community Health Advocates and outpatient providers to enroll patients, deliver MNT/DSME, and support population health goals. This PRN position offers 2-3 days per week and potential to impact food insecurity and health outcomes.
