Med Star Health is seeking a Population Health Dietitian to join the Food Rx program in Baltimore, MD. The role provides nutrition counseling, diabetes education, weight management strategies, and meal planning as part of a regional program addressing food insecurity and chronic disease management.
The position is PRN, 2-3 days per week, and collaborates with Community Health Advocates and interdisciplinary teams to enroll patients and integrate MNT/DSME services into care plans.
