Med Star Health is seeking a Population Health Dietitian to join the Food Rx program in Baltimore, MD. This role provides nutrition counseling, diabetes education, weight management strategies, and meal planning for individuals with chronic diseases and food insecurity.
The PRN position offers flexible 2–3 days per week, in collaboration with Community Health Advocates and the Collaborative Care Program to identify eligible patients, coordinate enrollment, and deliver MNT/DSME consults.
