Join Banner Home Health's Greeley team as a Physical Therapist and make a meaningful impact in patients' lives right in the comfort of their own homes! We're seeking a compassionate PT to work with our collaborative interdisciplinary team of nurses, therapists, and social workers serving the Greeley community. Whether you're an experienced home care therapist, have at least two years of PT experience in any setting, or are even a promising new graduate, we'd love to meet you!

In this rewarding role, you'll conduct evaluations, admissions, and treatments for diverse patient diagnoses, supervise PTAs, and enjoy the independence of managing your own schedule with a reasonable productivity expectation of 5-6 visits per day. We provide you with all the tools for success—including a work tablet, phone, access to ultrasound, e-stim, anodyne equipment, theraband, and federal mileage reimbursement—while you bring excellent communication skills, flexibility, strong documentation abilities, and a passion for delivering exceptional patient-centered care.

SCHEDULE

Full Time / 32 Hours Saturday and Sunday + additional days during the week 8:00am - 5:00pm

POSITION SUMMARY

This position is responsible for the evaluation, planning, administration and supervision of Physical Therapy treatment in accordance with physician referral, state and national organizational standards, regulatory agencies, and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.

CORE FUNCTIONS
  • Evaluates patient's functional abilities and limitations. Determines if intervention is needed. Establishes a plan of care and treatment goals. Completes comprehensive evaluation within the established time frame set by the department. Identifies and prioritizes key limiting factors. Establishes treatment goals that are functional, measurable, patient related and reflects the key limiting factors. Collaborates with patient and family/caregiver when setting goals. Initiates discharge planning.

    Recommends additions to or modifications of referring orders.
  • Implements individualized treatment plan with consideration to the patient's physical, social, spiritual, cultural, educational and age specific needs.
