Social Worker, Crisis Counselor

Heartland Hospice is seeking a compassionate Social Worker MSW to support patients and families facing life-limiting illness. In this role, you will complete psychosocial assessments, develop individualized care plans, and provide counseling, crisis intervention, and grief support in home and facility settings. You will collaborate closely with an interdisciplinary team, coordinate community resources, advocate for patient rights and goals of care, and assist with advance care planning.

Responsibilities

• Conduct comprehensive psychosocial assessments for hospice patients and families
• Develop and implement individualized psychosocial care plans
• Provide counseling, crisis intervention, and emotional support
• Offer grief and bereavement support to patients and caregivers
• Coordinate community resources and social services referrals
• Advocate for patient rights, preferences, and goals of care
• Collaborate with the interdisciplinary hospice team on care planning
• Assist with advance care planning and end-of-life decision support
• Maintain accurate, timely clinical documentation
• Participate in team meetings, case conferences, and quality initiatives
