Physical Therapist - Home Care

Job Summary

Provides evaluation and physical therapy treatment in compliance with state and regulatory standards.

• Analyzes patient responses, documents progress, revises care plans, and recommends updates based on interventions.
• Establishes and documents patient and family education plans, communicates effectively with patients and healthcare team, and schedules visits.
• Implements and adjusts treatment plans collaboratively, utilizing assessments like goniometry, muscle testing, and sensory evaluations.
• Independently manages care for routine to complex cases, maintains accurate clinical documentation, and submits reports timely.
• Participates in case conferences, multidisciplinary teams, community outreach, and continuous education.
• Performs assessments to determine rehabilitation needs, recommends adaptive equipment, and plans discharge and community referrals.
• Performs modalities such as therapeutic exercises, gait training, and heat/cold therapy.

• Bachelor’s degree in Physical Therapy from an accredited program.
• 1-2 years clinical experience.
• Valid Physical Therapist license, valid driver’s license, and CPR certification required.
• Strong clinical, organizational, communication, and computer skills, with knowledge of home care regulations.
