Occupational Therapist

We are seeking an Occupational Therapist in Home Health to assess and evaluate patients' functional needs related to daily activities, motor skills, home safety, and adaptive equipment.

• Providing services within legal scope and evidence-based practices, collaborating with the healthcare team
• Evaluating patients' functional status and developing care plans in consultation with physicians
• Monitoring and documenting patient responses to treatments and condition changes
• Conducting assessments using diagnostic and prognostic tests to determine functional ability
• Providing care regardless of age, race, creed, or socioeconomic status

• Current occupational therapy licensure in the practicing state
• Valid driver's license, vehicle insurance, and access to reliable transportation

• CPR certification or ability to obtain within 90 days
• Ability to handle multiple tasks independently with strong communication, organizational, and writing skills
