General Summary of Position

Manages the clinical pharmacy services. Ensures that the principles of pharmaceutical care are followed in the prescribing dispensing administration and monitoring of medications.

Primary Duties and Responsibilities
  • Ensures safe and appropriate medication use
    by reviewing medication orders for indication, dose, route, frequency, interactions, allergies, duplicate therapy, and other potential medication-related problems. Uses patient-specific clinical information, including diagnoses, medications, allergies, laboratory results, and clinical response, to support safe medication management.

  • Identifies, evaluates, and responds to medication-related risks and adverse events
    , including medication errors, adverse drug events, recalls, and other safety concerns. Analyzes medication occurrence data and trends to identify opportunities for improvement and supports required internal and external reporting.

  • Leads medication safety and quality improvement initiatives
    , evaluating medication-use processes for risk points and implementing evidence-based improvements, technologies, protocols, policies, and procedures to enhance safety, effectiveness, and efficiency.

  • Supports medication selection, procurement, and formulary management
    by evaluating medications based on safety, efficacy, clinical indication, and cost. Develops recommendations related to non-formulary medications, therapeutic substitutions, medication shortages, and emergency/disaster preparedness.

  • Ensures appropriate medication preparation, dispensing, labeling, storage, and disposal
    in accordance with organizational policies, laws, regulations, and standards of practice, including oversight of hazardous and controlled medications.

  • Develops and implements medication management policies, procedures, protocols, and education
    for patients, pharmacists, providers, and other clinical staff. Provides consultation regarding medication administration, adverse reactions, patient counseling, and other clinically significant medication concerns.

  • Monitors and improves medication management systems
    by evaluating utilization, reviewing emerging safety and efficacy information, maintaining appropriate medication storage and availability, and collaborating with healthcare providers and leadership to optimize medication safety.

Minimal Qualifications Education
  • Pharm. D required,2 years post-graduate Pharm.

    D. or equivalent preferred, orBCPS with a lesser degree required
Experience
  • 3-4 years Clinical hospital pharmacy experience with understanding of pharmaceutical care concepts and/or BCPS residency and management preferred
Licenses and Certifications
  • Pharmacist license in the State of Maryland required
Knowledge

Skills and Abilities
  • Verbal and written communication skills.
  • Basic computer skills preferred.
