The main purpose of this position is to coordinate services provided to individuals served by the Developmental Disabilities Administration (DDA) through the Developmental Disabilities Program (DDP) Targeted Case Management (TCM) and Coordinator of Community Services (CCS) programs. This position will coordinate available community resources for eligible individuals with developmental disabilities who are residents of Maryland's Eastern Shore. This position will assess and determine eligibility for DDA services and provide referrals for those services.

This position will arrange access to day, residential and other support services. This position will facilitate annual planning meetings and assist individuals and their families with developing a plan of service. This position will develop the Person-Centered Plan (PCP) and conduct periodic monitoring of the PCP and satisfaction with delivered services. This position will follow individuals on the waiting list registry and assist with transition planning.

This position will maintain accurate documentation of all services provided. These efforts will support individuals in maintaining community living, promoting individual choice and improving overall quality of life.

• (a) A Bachelor's degree from an accredited college or university in a Human Services field; OR
• (b) An Associate's degree with two (2) years experience in a Human Services field; OR
• (c) Seven (7) years experience in a Human Services field.

• Education:
  
  Graduation from an accredited high school or possession of a high school equivalency certificate.
• Experience:
  
  One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
• Notes:
  • 1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
  • 2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

• 1. Experience working with individuals who have intellectual or developmental disabilities.
• 2. Experience with case management.
