Role Description

We’re building a world of health around every individual — shaping a more connected, convenient and compassionate health experience. Oak Street Health seeks a Nurse Practitioner to provide effective, equitable value‑based primary care to Medicare adults, working independently while collaborating with physicians and a multidisciplinary care team.

Core Responsibilities
  • Provide exceptional primary care.
  • Conduct office visits for routine and acute issues.
  • Administer annual wellness visits and health risk assessments with a holistic approach and thorough documentation.
  • Coordinate care with other providers, specialists, testing facilities, and agencies.
  • Lead population health initiatives, ensuring eligible patients receive evidence‑based screenings (e.g., breast cancer screening every two years).
  • Assist the care team with phone triage and outreach.
  • Educate patients on health conditions, care plans, and treatments.
  • Participate in Oak Street Health promotional activities.
  • Conduct home visits as needed.
  • Perform other duties as assigned.
Required Qualifications
  • Graduate of a Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program.
  • National certification in at least one specialty:
    Family NP, Adult‑Gerontology Primary Care NP, Adult NP, or Gerontological NP.
  • Active, non‑probationary state Nurse Practitioner license.
  • Active DEA license.
  • US work authorization.
Preferred Qualifications
  • Experience in primary care fields such as internal medicine, geriatrics, or family medicine.
  • Knowledge of HCC documentation, ICD‑10 coding, and Health Risk Assessments.
  • Passion for teamwork and cross‑functional collaboration.
  • Desire to participate in an innovative, empirically‑guided population health model.
  • Bilingual proficiency where applicable.

Pay Range: $78,331.00 – $ (base salary varies by experience, education, geography, and other factors).
