Role Description

Oak Street Health seeks a Nurse Practitioner to provide effective, equitable value‑based primary care to Medicare adults, working independently while collaborating with physicians and a multidisciplinary care team.
Core Responsibilities- Provide exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer annual wellness visits and health risk assessments with a holistic approach and thorough documentation.
- Coordinate care with other providers, specialists, testing facilities, and agencies.
- Lead population health initiatives, ensuring eligible patients receive evidence‑based screenings (e.g., breast cancer screening every two years).
- Assist the care team with phone triage and outreach.
- Educate patients on health conditions, care plans, and treatments.
- Participate in Oak Street Health promotional activities.
- Conduct home visits as needed.
- Perform other duties as assigned.
- Graduate of a Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program.
- National certification in at least one specialty:
Family NP, Adult‑Gerontology Primary Care NP, Adult NP, or Gerontological NP. - Active, non‑probationary state Nurse Practitioner license.
- Active DEA license.
- US work authorization.
- Experience in primary care fields such as internal medicine, geriatrics, or family medicine.
- Knowledge of HCC documentation, ICD‑10 coding, and Health Risk Assessments.
- Passion for teamwork and cross‑functional collaboration.
- Desire to participate in an innovative, empirically‑guided population health model.
- Bilingual proficiency where applicable.

Pay Range: $78,331.00 – $ (base salary varies by experience, education, geography, and other factors).
