Job Summary:
Join our team at South Mountain Rehab Center, a warm and home-like environment where personalized care is our top priority. As a Licensed Practical Nurse (LPN), you will provide supervision of staff and safeguard the health, safety, and welfare of all residents under your care. We"re looking for a dedicated individual with a passion for delivering high-quality care to join our Skilled Nursing team.
Responsibilities- Deliver safe, timely, and effective nursing care to residents under the direction of an RN or physician
- Monitor patients' health, administer medications, and provide wound care and other treatments
- Collaborate with interdisciplinary teams to develop and update individualized care plans
- Support residents and their families with education, comfort, and reassurance
- Maintain accurate documentation and comply with regulatory standards
- Follows the plan of care as developed by the RN staff nurse, MDS Coordinator(s), or RN members of the Nursing leadership team
- Observes safety policies and reports any safety concerns
- Demonstrates respect of resident rights and dignity
- Fulfills the role of charge nurse when assigned
- Supervise and support CNAs in the delivery of care
- Maintain a clean, safe, and therapeutic environment for residents
- Ensure compliance with infection control, safety, and HIPAA policies
- Participate in quality assurance activities and staff meetings
- Active LPN license in Maryland or Multistate
- Experience in long-term care or skilled nursing preferred – new grads welcome!
- Strong clinical skills, attention to detail, and a heart for service
- Ability to thrive in a fast-paced, team-oriented environment
- Excellent communication and interpersonal skills
- Competitive pay: $27 - $34 HOURLY
