The LPN is responsible for providing quality patient care and performing skilled care in the patient’s home. The role includes:

• Providing services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other members of the health care team.
• Instructing the patient’s family in the care of the patient and maintaining a healthy environment for the patient.
• Observing, recording, and reporting the patient’s response to treatment and changes in the patient’s condition daily to the supervising nurse and/or physician.
• Collaborating closely with physicians and RNs, documenting care accurately and promptly, and not altering the plan of care.

• One year of clinical LPN experience.
• Current LPN license in the state where services are rendered.
• Current CPR certification.
• Driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Reliability in commuting to multiple patient residences daily within the geographic area assigned.
