About the Role

The Clinical Program Coordinator will successfully complete hospital and nursing orientation at CNMC and coordinate nursing care and services for patients and families in various encounters (telephonic, electronic, in person, mail). They will consult with physicians and other providers to resolve health concerns and ensure all nursing practice aligns with legal scope, national and specialty standards, CNMC policies, and applicable laws.

The coordinator will lead the patient care delivery team for specific populations, assess and plan clinical care, and maintain continuity of care.

Responsibilities
  • Support shared nursing leadership and contribute to the sustainability and advancement of nursing excellence, including Magnet designation.
  • Participate in and contribute to internal/external committees, professional organizations, and community activities.
  • Support the Division of Nursing’s performance improvement process and identify, define, and report on measures consistent with Nurse Sensitive Indicators (NSIs) for ambulatory nursing practice.
  • Integrate evidence‑based practice and research into daily care.
  • Assist physicians and other providers to maintain appropriate cost efficiencies while achieving desired patient outcomes.
  • Speak up about unsafe behavior, verify information, and resolve uncertainties promptly.
  • Communicate accurately and promptly in both verbal and written formats.
  • Actively promote safety for patients, families, visitors, and coworkers.
  • Plan and collaborate with patients, families, and providers for shared decision‑making and advocacy.
  • Provide comprehensive health education tailored to patient/family learning preferences throughout the patient’s health trajectory.
  • Implement nursing processes in clinic and telehealth settings to assess patient problems and integrate data.
  • Coordinate care across systems and institutions and develop patient care policies and protocols.
  • Maintain and ensure safe use of medical equipment and supplies.
  • Adhere to regulatory requirements and keep updated on health informatics applications.
  • Apply operations, financial, and resource principles to deliver cost‑effective and safe care.
  • Promote collaboration and maintain high‑performance team environments.
  • Facilitate interdisciplinary patient‑care rounds to review goals and optimize outcomes.
  • Manage scheduling, coverage, and flow of patients and staff to assure smooth care delivery.
  • Collaborate with multidisciplinary staff in accordance with state practice acts and CNMC job descriptions.
  • Escalate clinical or administrative issues as appropriate.
  • Participate in peer review of staff.
Qualifications
  • Minimum Education: Bachelor’s Degree (Required)
  • Minimum

    Work Experience:

    • 5 years nursing experience (Required)
    • Minimum one year recent pediatric experience in a comparable setting (Required)
    • Minimum one year of nursing leadership experience (charge nurse, educator, preceptor, council chair/member, or comparable) (Required)
  • Registered Nurse with full scope of practice and professional accountability.
