The RN provides and directs nursing care to patients in their homes as prescribed by the physician, complying with applicable laws, regulations, and agency policies.

• Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization.
• Maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources.
• Conduct initial and/or comprehensive nursing evaluation visits, determine primary focus of care, develop the plan of care within state-specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
• Maintain and submit accurate documentation and clinical notes, including patient condition, goals, interventions, and response to care.
• Establish and maintain an effective and efficient visit route.
• Provide high-quality clinical services within the scope of practice defined by state laws, in accordance with the plan of care, from admission through discharge.

• One (1) year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification.
• Driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation; reliable commute to multiple patient residences daily within the assigned geographic area.
