Full‑time, 40 hours per week. 3‑12‑hour weekend shifts rotating every 4‑5 weeks. On‑call rotation 5p.m.–8p.m. (usually included in shift).

• Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Review patient history, physical examination, diagnostic and laboratory data; report abnormal findings to physicians.
• Formulate and revise individualized plans of care, including discharge plans, in collaboration with multidisciplinary team.
• Implement plans of care through direct patient care, coordination, delegation, and supervision of other health‑care team members.
• Document assessment, planning, implementation, and evaluation in the patient record; maintain legible, timely documentation.
• Provide care based on evidence‑based best practice; participate in unit and facility shared leadership, education, orientation, and mentoring.
• Contribute to quality improvement, research, and professional organization participation to improve patient outcomes.
• Supervise staff and workflow when required; interact with all levels of staff, physicians, patients, families, and external contacts including clergy.

• Current valid RN license in the state of practice, a temporary/interim RN license, or compact RN licensure.
• BLS certification.
• Experience in hospice, home health, or med‑surg; clinical competency demonstrated through annual validation.
• For acute‑care positions: minimum one year experience in an acute‑care hospital setting within the past 12 months.
• Home‑health role requires valid driver’s license, personal vehicle, and active auto insurance.
• Arizona positions require appropriate fingerprint clearance cards and a criminal history affidavit as applicable.

• Bachelor’s degree.
• Professional certification in related specialty.
• Additional education or experience preferred.

$33.11 – $55.18 per hour. The range reflects possible base salaries; the total rewards package is not included.
