Responsibilities- Provide quality nursing care and services to residents in a respectful and dignified manner, ensuring the delivery of care and services.
- Coordinate the physical, social, and emotional aspects of assisted living care in accordance with physician orders and resident choices.
- Execute the medication management plan, including periodic review of documentation, observation of effectiveness, and communication with the physician when changes are needed or outcomes are inappropriate.
- Continuously assess resident needs with updates to appropriate individuals regarding care, services, and changes in condition.
- Implement measures to ensure resident wellness and safety, and document those measures appropriately.
- Work in a team fashion to meet resident needs.
- Respond to emergencies timely, calmly, and competently.
- Report incidents and changes in resident condition to the Healthcare Director with appropriate documentation.
- Participate actively in all activities, including community events.
- Assist with delivery of meals in the dining room, monitor resident acceptance of diets, and ensure adequate nutrition.
- Maintain high standards of record keeping, resident, and staff documentation.
- Establish and maintain a positive working relationship with all departments.
- Identify risk factors for residents and initiate interventions, documenting those interventions appropriately.
- Provide a thorough report of resident status to the incoming nursing team at each shift change and in the Daily Log.
- Provide motivation and direction to CNAs/RMAs.
- Adhere to policies, procedures, body mechanics, and safe resident transfer techniques.
- Adhere to attendance policies; report to work on time and prepared to begin duties.
- Complete all other assigned duties.
- Current LPN license to practice nursing in the state of employment, in good standing with the Board of Nursing.
- Experience working with the senior population, preferably in assisted living or long‑term care.
- Team player with experience supervising staff.
- Critical thinking skills and effective verbal and written communication.
- Demonstrated ability to manage time efficiently with strong organizational skills.
- In good physical condition, able to sit, stand, bend, kneel, walk, and lift up to 50 lbs.
- Willingness to work an 8–16 hour flexible schedule, including weekends and holidays.
- Attendance at required trainings, CEUs, and staff meetings.
- Certification or willingness to take and pass the required state certification class.
