RN Registered Nurse - Personal Care

Stella Maris is proud to provide central Maryland with compassionate, personal care for adults. We specialize in home health care services that enable the patient to adapt and manage his or her medical situation in the comfort of their home.

Stella Maris Personal Care Department has an opening for a Per Visit-Registered Nurse to perform personal care home visits. This position will cover clients on a per visit basis in territories throughout Harford County, Baltimore County, and Baltimore City.

Registered Nurses (RN)- Personal Care

Per Visit/Flex/PRN/As Needed Visits

• Makes initial and follow‑up visits to patients assigned.
• Schedules visits independently by contacting family/client directly based on list provided by department management via Epic/Rover.
• Demonstrates excellent customer service with clients, coworkers and administration.
• Uses the nursing process of assessment, medication lists and evaluation to provide physical and psycho‑social care to patients and their families.
• Escalates concerns as appropriate for additional needs of client.
• Maintains ongoing communication as necessary to revise the plan of care to assure quality home care.
• Teaches and supervises CNA/GNA/Med Tech in providing care to patients and evaluates the care they provide, including medications, simple dressing changes, use of equipment and psychosocial support.
• Documents accurately all visits and phone calls; completes accurately all documentation in Epic/Rover system.
• Supervises home health aides in patients’ homes in accordance with State regulations and documents same on patient record.
• Completes all documentation as required by agency standards, regulatory standards and standards of nursing practice.
• Completes all assigned in‑services as well as attends meetings as scheduled.

• RN licensure in the State of Maryland
• CPR Certification
• Prior experience in home health required
• Valid driver’s license and reliable transportation
