Sign On Bonus: $5,000

Schedule: Full-time, Monday-Friday 8:00 a.m. – 4:30 p.m.

Under the direction of the clinical manager, the Home Health Registered Nurse performs primary nursing care functions in conformance with professional standards of home health nursing practice.

• Provide individualized care to patients in their homes under the plan of care approved by the primary physician.
• Coordinate all services provided for each assigned patient and family.
• Provide leadership and advocacy in the coordination of patient care with multiple clinicians, modalities, and community resources.
• Manage clinical resources and transition planning for patients within the assigned caseload from pre‑admission through post‑discharge, actively working to provide excellent medical care.
• Travel daily within the assigned region.

• RN licensure in the State of Maryland required.
• Prior experience in hospice or home health required.
• OASIS Documentation experience preferred.
• Valid driver’s license and reliable transportation.

Minimum salary: USD $38.05 per hour.

Maximum salary: USD $57.05 per hour.

Compensation decisions are made in alignment with internal equity, role scope, and market data to ensure fair and consistent pay practices. Most new hires are typically placed within the lower to mid‑portion of the range; offers above that level are reserved for candidates with advanced or highly specialized qualifications.
