Licensed Practical Nurse – Wexford Health
Position Summary
As an LPN at our facility, you’ll play a hands‑on role in delivering care to a vulnerable population under the guidance of RNs and medical providers.
Some responsibilities include:
- Administering medications and treatments as ordered
- Assisting with activities of daily living and therapeutic interventions
- Taking vital signs and monitoring patient conditions
- Providing wound care and changing dressings
- Supporting documentation and care planning
- Promoting hygiene, sanitation, and a safe environment for all patients
- Serving as a vital part of a multidisciplinary treatment team

What You’ll Need
- Active unrestricted LPN license in state of practice.
- 1‑2 years of nursing experience (correctional experience preferred).
- Active CPR certification.
- Strong clinical and communication skills; adaptability in high‑pressure settings.
- A compassionate, nonjudgmental approach to patient care.

Key Responsibilities:
- Administer medications, treatments, and wound care under RN supervision.

- Maintain accurate clinical records, including medication logs and shift reports.
- Ensure compliance with infection control, safety protocols, and facility policies.
- Provide emotional support and advocate for the patient’s needs during rehabilitation.
Requirements
Qualifications:
- Education:
Completion of an accredited Licensed Practical Nurse program. - Licensure:
Active MD state LPN license and current BLS Certification. - Experience:
At least one year of relevant field experience is required. - Soft Skills:
Compassion, reliability, strong communication skills, attention to detail, and the ability to work effectively as part of a healthcare team.
