Candidate Type:Local or Travel
Requirements: 1 year of Medicare certified Home Health experience and HCHB EMR experience

• Provide skilled nursing care to patients in the home health setting while following established care plans.
• Complete thorough patient assessments and document findings, interventions, and patient responses.
• Communicate effectively with patients, families, and the care team to support continuity of care.
• Educate patients and caregivers on care needs, medications, and safety considerations as appropriate.
• Maintain accurate, timely documentation in accordance with assignment and facility standards.
