Catholic Charities of Baltimore, Anna's House is seeking a Case Manager to provide intensive case management, including as‑needed home visitation services to participants and clients. The Case Manager assesses household needs, coordinates resources based on goals, and provides advocacy, information, and referral services. The work schedule is Monday‑Friday 9 AM – 5 PM with one evening shift from 12 PM – 8 PM.

Anna's House provides eight onsite transitional living spaces to the broad array of homeless services it offers, including emergency shelter, transitional housing for families fleeing domestic violence, community supportive housing, and rapid re‑housing. It supports each family or individual to take charge of their life and recognize their own capabilities and self‑worth, moving toward self‑sufficiency.

Responsibilities
  • Develop Individualized Service Plans (ISPs) in collaboration with participants, focusing on achieving self‑sufficiency within established timelines, and track progress to ensure goal completion.
  • Conduct thorough self‑sufficiency assessments at intake and service closure using the Arizona Self‑Sufficiency Matrix; document and communicate results to inform service planning.
  • Maintain consistent contact and conduct required visits to evaluate progress, address challenges, and ensure participant engagement and compliance with the ISP.
  • Advocate effectively for individuals, families, and children, securing needed services within the community and following up to confirm support is received.
  • Collaborate with staff and volunteers to engage clients in programs that support their path to self‑sufficiency, taking initiative to align services with individual goals.
  • Maintain accurate case files, enter data into the Service Point database in a timely manner, and ensure data integrity to support reporting and analysis.
  • Assist clients in accessing and completing applications for eligible benefits such as emergency cash assistance, SNAP, day‑care vouchers, and eviction prevention; follow up to track outcomes and provide further support as needed.
  • Collect, input, and report data per program requirements; keep supervisors informed of trends, issues, and participant outcomes.
  • Utilize Motivational Interviewing and strength‑based coaching to help clients overcome barriers and take actionable steps toward their desired outcomes.
  • Foster an environment aligned with Trauma‑Informed Care (TIC) principles by maintaining safety, trust, and emotional support in all client interactions.
  • Participate in all required meetings, trainings, and program events, including graduation ceremonies, and apply learned concepts to improve client outcomes.
  • Maintain a working knowledge of program services offered and connect clients to internal resources, reinforcing comprehensive support toward self‑sufficiency.
  • Perform other duties as assigned.
Program‑Specific Duties
  • Participate in local Continuum of Care Meetings and Committees.
  • Facilitate Life Skills classes and provide after‑care services to clients for up to twelve months.
  • Provide direction to Program Assistants in support of case management.
  • Housing Services
    • Ensure compliance with U.S. Department of Housing and Urban Development (HUD) rules and regulations and pass annual HUD audits and other privately funded programs.
    • Coordinate housing leased to or for clients, negotiate leases with landlords and occupancy agreements with residents; maintain ongoing contact with realty representatives and property managers.
    • Inspect the property and/or apartments at specified intervals.
    • Reinforce occupancy policies and procedures by initiating procedures for non‑payment of rent.
    • Monitor the upkeep of the property, including snow and trash removal, landscaping, and routine maintenance and repairs.
Education and Experience Requirements
  • Bachelor’s degree in Human Services, Social Work, or a related field.
  • Two years of related experience.
  • Excellent knowledge of case management and crisis intervention services.
  • An equivalent combination of education and experience may be considered.
